Illinois Form IL-501

This article explains what 2026 Illinois Form IL-501 is, who should use it, when withholding tax payments are due, and how to complete every field on the payment coupon correctly.

Illinois Form IL-501, Payment Coupon, is a payment document issued by the Illinois Department of Revenue for businesses and employers that need to submit Illinois income tax withheld from employees or other payees. It is used to accompany a payment of withheld Illinois income tax when an employer qualifies to use a paper payment coupon. The form is intended for limited use and is not the standard payment method for most employers. Businesses that have recently started operating and have not yet received their Illinois tax registration information may use it temporarily. Employers who have ordered personalized payment coupons but have not received them may also use this form. The coupon collects the employer’s Federal Employer Identification Number (FEIN), sequence number, business name, mailing address, telephone number, applicable tax year, reporting quarter, and payment amount. It also identifies the months included in the selected quarter so the Illinois Department of Revenue can associate the payment with the correct withholding period. Employers must report payments made using Form IL-501 on Form IL-941, Illinois Withholding Income Tax Return, in Step 5, Line 4, for the corresponding quarter. The amount entered on the coupon must reflect the actual Illinois income tax withheld from employees or other payees. Illinois considers income tax withheld on the date the employer pays the employee or other recipient, making the payment date important when determining when the tax must be submitted. Most employers are encouraged to use electronic payment services, while employers assigned to a semi-weekly payment schedule are required to pay electronically. Completing Form IL-501 accurately helps ensure that payments are properly identified, credited to the correct tax account, and reported for the appropriate quarter.

Who Should Use Form IL-501?

Form IL-501 is intended for a limited group of Illinois employers who need to make withholding income tax payments but cannot yet use their regular personalized payment coupons.

You should generally use this coupon only if one of the following situations applies:

  • New Business: Your business is newly registered and you have not yet received your Illinois tax registration information.
  • Personalized Coupons Not Received: You have already ordered personalized Illinois withholding payment coupons but have not received them.

Most other employers should make their withholding payments electronically or use the personalized payment coupons provided by the Illinois Department of Revenue.

Employers Required to Pay Electronically

If the Illinois Department of Revenue has assigned your business to a semi-weekly payment schedule, you must make withholding tax payments electronically.

Employers assigned to a monthly payment schedule are also encouraged to use electronic payment methods.

Do not use Form IL-501 as a substitute for required electronic payments.

How To File Form IL-501

Form IL-501 is submitted with the payment of Illinois income tax withheld from employees or other payees. It is not a replacement for Form IL-941, Illinois Withholding Income Tax Return.

The payment coupon identifies the amount being submitted and the reporting period to which the payment belongs.

Where to Mail Form IL-501

If you are paying by check, mail the completed coupon and payment to:

Illinois Department of Revenue
PO Box 19447
Springfield, IL 62794-9447

Make your check payable to Illinois Department of Revenue.

Write your FEIN and the words “IL-501” on the check so the payment can be identified correctly.

Do not mail Form IL-501 if you make your payment electronically or if you are reporting a zero payment.

The Fed/State Employment Taxes (FSET) Program is another electronic payment option. It is designed for large employers and payroll companies.

Employers can review the available electronic payment methods through the Illinois Department of Revenue’s online services.

Requesting Personalized Payment Coupons

You can request that personalized coupons be mailed to your business using either of these telephone numbers:

  • 1-800-732-8866

Information to Prepare Before Completing Form IL-501

Gather the following information before filling out the payment coupon:

  • Employer’s Federal Employer Identification Number (FEIN).
  • Sequence number, if assigned.
  • Legal business name.
  • Business mailing address.
  • City, state, and ZIP code.
  • Daytime telephone number.
  • Tax year.
  • Calendar quarter in which the withholding occurred.
  • Total Illinois income tax withheld.
  • Payment amount being submitted.

You should also review your payroll records to determine the exact amount of Illinois income tax withheld during the applicable reporting period.

When Are Illinois Withholding Tax Payments Due?

The payment deadline depends on the payment schedule assigned to your business by the Illinois Department of Revenue.

Monthly Payment Schedule

Most employers who use paper payment coupons are classified as monthly payers.

Monthly payers must submit their Illinois withholding tax payments by the 15th day of each month for taxes withheld during the preceding month.

For example, Illinois income tax withheld from employees during January must generally be paid by February 15.

Employers may choose to pay more frequently through electronic payment methods or preprinted personalized payment coupons.

However, do not use Form IL-941 to make more frequent withholding tax payments. The return is used for reporting the withholding tax rather than making additional interim payments.

Semi-Weekly Payment Schedule

Employers assigned to a semi-weekly payment schedule must make their payments electronically.

These employers cannot use Form IL-501 to submit paper payments.

Follow the electronic payment requirements and deadlines applicable to your assigned schedule.

Important: When Is Income Tax Considered Withheld?

Illinois considers income tax withheld on the date an employer pays an employee or another payee.

This date determines when the withholding tax payment becomes due.

For example, if an employer pays an employee on January 31, the Illinois income tax withheld from that payment is considered withheld on January 31.

Use the actual payment dates when determining your withholding obligations.

Do not estimate the amount of withholding tax.

How Much Should You Pay?

The amount entered on Form IL-501 must equal the Illinois income tax actually withheld from employees or other payees.

Your payment amount depends on the withholding amounts calculated from payroll and other applicable payments.

Use the 2026 Booklet IL-700-T, Illinois Withholding Tax Tables, to determine the correct withholding amounts.

Do not enter an estimated payment based on expected payroll or anticipated withholding.

Reporting Form IL-501 Payments on Form IL-941

Employers must report all payments made using Form IL-501 on Form IL-941, Illinois Withholding Income Tax Return.

Form IL-941, Step 5, Line 4

Enter the total Form IL-501 payments made for the corresponding quarter on Step 5, Line 4, of Form IL-941.

Make sure the amount reported agrees with the payment records for that quarter.

Keep your payment records so you can verify that the amounts reported on Form IL-941 agree with the amounts actually submitted.

Important Rules to Remember When Using Form IL-501

Form IL-501 Is for Limited Use Only

Semi-Weekly Employers Must Pay Electronically

Monthly Payments Are Generally Due by the 15th

Most monthly payers must pay by the 15th day of the month following the month in which the tax was withheld.

Follow the payment schedule assigned to your business.

Do Not Submit a Zero-Payment Coupon

If no Illinois income tax was withheld, do not make a payment or submit Form IL-501.

Do Not Estimate Withholding Tax

Use actual payroll and payment records to determine the withholding amount. Illinois considers the tax withheld on the date employees or other payees are paid.

Report All Payments on Form IL-941

Every payment made using Form IL-501 must be reported on Form IL-941, Step 5, Line 4, for the applicable quarter.

Do Not Use Form IL-941 for More Frequent Payments
